It happened in a Maytag top loader in the 1980's, the lid switch failed in the closed position, that's the reason they added the check switch and fuse to the lid switch. Maytag got sued for somebody's dislocated arm.

looking at the video I would say that the center shipping bolt was never removed.that might have something to do with what happened to this poor girl.

We get a lot of machines in for service that have had the lid switch deliberately bypassed. We either make them agree to have it replaced or have them sign releasing us from any legal action.
